Please include your name, birthday, address, and Advantage number, if possible. If you would like your statement faxed, please include your fax number (including area code). Guests under the age of 21 are not allowed access to any gaming area of the casino. Coushatta Casino Resort will check the ID's of those guests who appear to be under the age of 21.
A valid state-issued photo ID or valid passport is acceptable as proof of age. If your valid driver's license has your photo on it, it is also acceptable. Other identification that will serve as proof of age include your military identification, Merchant Marine identification, international driver's license, or resident alien registration (green card).
If you need a wheelchair, go to the casino valet or hotel valet booth for assistance. There is no charge for its use. Your valid photo ID will be held at the location until you return the wheelchair.
